• Application/Essay Form
  All prospective students need to complete an application.

• Transcript Release Form
  All prospective students must have their CURRENT school fill out this form and return it to STA.

• English Evaluation Form
  All prospective students must have their current English teachers complete and return this form. It will NOT become part of the student's file.

• Math Evaluation Form
  All prospective students must have their current math teachers complete and return this form. It will NOT become part of the student's file.
