Required Athletic Forms

For all students in grades 7-12.
A physical exam is required every three years.

Every student has to complete the MSHSHL Eligibility Brochure at the beginning of each school year.
 
Every student must have a current Physical Examination Form on file with the Health Office.

* NEW EFFECTIVE JUNE 27, 2011

Recently-passed Minnesota statutes have added a new requirement regarding the nature and risks of concussions. This form must be signed by the student and a parent, and returned to school, before the student will be allowed to participate in athletics.
