Required Athletic Forms
For all athletes participating in a High School League sport grades 7-12.
A physical exam is required every three years.
Triplicate Information Form is only available in "paper" from Mr. Zahr or Ms. Marks. This form is used by the athletic office and coaches to track the necessary paperwork.
The Athletic Emergency Card and Triplicate Information Form must be filled out for every new sport that a student tries out for.
Every student has to complete the MSHSHL Eligibility Brochure once per school year, before beginning his first sport.
Every student must have a current Physical Examination Form on file with the Nurse’s Office.
