• 2017-18 Consent to Share/Medications/Administration

This form needs to be filled out every year for every student. Information includes authorizing the school health staff to share health concerns with appropriate school personnel who have direct contact with your son during the school day; listing your son's medications and allergies; and providing your consent for the health staff to administer Ibuprofen/Tylenol.

• 2017-18 MSHSL Eligibility Brochure

Every student must read and complete the MSHSL Eligibility Brochure each year they are enrolled at STA. Students who have not completed the Eligibility Brochure and/or do not have a current physical examination and current immunization records on file are unable to participate or practice any MSHSL activity. Immunization records are required for enrollment.

• Qualifying Physical Examination

A physical exam is required every three years. Every student must have a current Physical Examination form on file with the Health Office.

Lunch options—The Taher program or lunch from home—was included in your contract and billed through SmartTuition.

Vehicle Parking Permit—Returning students need to complete the form below only if they will be driving a different vehicle than last year. Parking permits will be available for pick up in the main office. Saint Thomas Academy does not charge for parking permits, however, fines will be assessed to students who do not register their vehicles.
