For Students in grades 6-12

 

• 2017-18 Consent to Share/Medications/Administration

This form needs to be filled out every year for every student - grades 6-12. Information includes authorizing the school health staff to share health concerns with appropriate school personnel who have direct contact with your son during the school day; listing your son's medications and allergies; and providing your consent for the health staff to administer Ibuprofen/Tylenol.

Every student - grades 7-12 only - must complete the MSHSL Eligibility Brochure at the beginning of the school year.

• Qualifying Physical Examination

A physical exam is required every three years - grades 6-12.
All enrolled students are required to have on file in the STA Health Office a physical examination that was completed within the last three years. Physical examinations are required for both enrollment (physical education and high school Military/Leadership classes) and participation in Minnesota State High School League (MSHSL) or Catholic Athletic Association (CAA) athletics/activities. Our school nurse will notify families when a student’s physical examination will need to be updated.

EVERY THREE YEARS: Physical Exam or send one from your doctor to nurse Lucy Poole at lpoole@cadets.com.

This form can be used if you do not have one from your doctor. Immunization records are required for enrollment.
